Revenue code 0900 is the general behavioral health revenue code on institutional claims, and it is one of the most misused codes on the UB-04. Behavioral health claims come back when the revenue code doesn’t match the service, the HCPCS code or the program type. An IOP day billed like a therapy visit, an FQHC mental health line on the wrong revenue code, or a 090X code on a CMS-1500 all stop payment. On the behavioral health claims we work, most of these errors are simple to prevent once you know which code goes where.
This guide covers revenue code 0900, the full 090X and 091X list, how IOP and PHP claims are coded, the FQHC rules, and how to fix the denials these codes cause.
Quick Answer
Revenue code 0900 is the general classification for behavioral health treatments and services on institutional claims (UB-04 / 837I). The 090X and 091X codes list specific services such as IOP, PHP, individual, group and family therapy, and testing. Medicare FQHCs report mental health visits G0469 and G0470 on revenue code 0900.

What Is Revenue Code 0900?
Revenue codes are 4-digit codes that tell a payer where a service happened or what type of service it was on an institutional claim. They are maintained by the National Uniform Billing Committee (NUBC) and go in form locator 42 of the UB-04 (loop 2400, SV2 segment on the 837I).
Revenue code 0900 is the general classification in the behavioral health family. It describes behavioral health treatments or services without naming a specific type. The 090X and 091X codes around it name the specific service: intensive outpatient, partial hospitalization, individual therapy, testing and more. For another common UB-04 code, see our guide to revenue code 0022 for SNF claims.
Who uses revenue code 0900?
- Hospital outpatient departments (TOB 013X) for behavioral health services such as psychiatric evaluations
- Community mental health centers (CMHCs) (TOB 076X)
- Federally qualified health centers (FQHCs) (TOB 077X) for Medicare mental health visits
- Psychiatric and substance use facilities that bill Medicaid or commercial payers on the UB-04
Watch Out: Revenue code 0900 belongs on institutional claims only. A therapist in private practice who bills on the CMS-1500 does not use revenue codes at all. Putting a revenue code on a professional claim usually leads to a rejection.
Behavioral Health Revenue Codes 090X and 091X: Full List
The 091X series is an extension of 090X, so payers treat them as one behavioral health family. The table below gives a plain-English meaning for each code. The official descriptions are in the NUBC UB-04 Data Specifications Manual.
| Code | Plain-English meaning | Typical use | Claim type |
|---|---|---|---|
| 0900 | Behavioral health treatments/services, general | Psychiatric evaluation (90791/90792); FQHC mental health visits (G0469/G0470) | UB-04 / 837I |

Meanings for 0900, 0904–0906 and 0914–0918, and the HCPCS examples, follow First Coast’s Medicare IOP billing guidance. Other descriptions come from common code lists; confirm every code in the current NUBC manual.

When to Use Revenue Code 0900 vs a Specific 090X/091X Code
The general rule on the UB-04: use the most specific revenue code that fits the service. Revenue code 0900 is the right choice when:
- A payer tells you to use it for a specific service, such as Medicare’s FQHC mental health visits or a psychiatric evaluation.
- The service is a behavioral health service with no specific code in the family.
Use a specific code when one fits. Individual psychotherapy belongs on 0914, group therapy on 0915, and testing on 0918. A payer edit may deny or return a line when revenue code 0900 is used for a service that has its own code, or when the HCPCS code doesn’t match the revenue code.
Pro Tip: Build a crosswalk in your billing system that ties each CPT or HCPCS code to the revenue code each payer wants. Medicare, Texas Medicaid and commercial plans often expect different revenue codes for the same service.
Revenue Code 0900 in FQHCs
For Medicare FQHC claims (TOB 077X), the revenue code is not optional. The Medicare Claims Processing Manual, Chapter 9, sets these rules:
- G0469 (new patient mental health visit) and G0470 (established patient mental health visit) go on revenue code 0900.
- G0466, G0467 and G0468 (medical visits and IPPE/AWV) go on revenue code 052X.
- Revenue code 0519 replaces 052X and 0900 only on Medicare Advantage supplemental (wraparound) claims.
- Every G-code line must be followed by a qualifying visit line, such as 90791, 90834 or 90837, on the same revenue code.
FQHCs that run a Medicare IOP use revenue code 0905 with condition code 92 (CMS MM13264), not revenue code 0900. For the full payment rules, see our FQHC PPS rate guide.

IOP and PHP Revenue Codes
Intensive outpatient (IOP) and partial hospitalization (PHP) programs cause more revenue code errors than any other behavioral health service. Medicare and other payers code them differently.
| Program | Revenue codes | Condition code | Type of bill | Who bills |
|---|---|---|---|---|
| Medicare IOP, hospital/CMHC/CAH | Service lines such as 0905, 0906, 0900, 0904, 0914–0916, 0918, 0942, 043X, 0250 | 92 | 013X hospital, 076X CMHC, 085X CAH | Hospital outpatient, CMHC, CAH |
| Medicare IOP, FQHC/RHC | 0905 | 92 | 077X FQHC, 071X RHC | FQHC, RHC |
| Medicare PHP | Service lines such as 0904, 0914–0916, 0918, 0942, 043X, 0250 | 41 (hospitals) | 013X hospital, 076X CMHC | Hospital outpatient, CMHC |
| Commercial / Medicaid IOP | Often 0905 or 0906 as a per diem | Per payer | Per payer | Per contract |
| Commercial / Medicaid PHP | Often 0912 or 0913 as a per diem | Per payer | Per payer | Per contract |
Medicare IOP: CMS MM13222, MM13264 and First Coast IOP billing requirements. Medicare PHP: First Coast PHP coding guideline. Commercial and Medicaid codes vary by payer; check the contract or provider manual.

Key rules for Medicare IOP and PHP claims:
- Hospitals and CMHCs must report a valid HCPCS or CPT code on each IOP line. CAHs don’t have to for IOP (First Coast).
- An IOP claim with condition code 92 that overlaps a PHP claim within 7 days is returned to provider (reason code 38363).
- Medicare’s own IOP and PHP claims don’t depend on revenue codes 0912 and 0913. Those codes show up mostly on commercial and Medicaid per diem claims. [VERIFY: current Medicare PHP revenue code list in your MAC’s PHP billing article]
For the full program rules, the 9-hour weekly minimum and 2026 rates, read our IOP billing guide. If your program bills IOP or PHP to several payers, our behavioral health billing services team sets up the right code set for each one.
How to Code a Behavioral Health Line on the UB-04: 6 Steps
- Confirm the claim is institutional. Revenue codes go on the UB-04 / 837I only. If the provider bills on the CMS-1500, use CPT/HCPCS and place of service instead.
- Identify the program or service. Is it a single visit, an IOP day, a PHP day, testing or an FQHC encounter? The program type decides the code set.
- Choose the revenue code. Pick the most specific 090X/091X code, or revenue code 0900 when the payer requires it (for example, FQHC G0469/G0470).
- Add the HCPCS or CPT code the payer requires. Examples: 90837 on 0914, G0470 on 0900 for an FQHC, H0015 or S9480 on a commercial or Medicaid IOP line.
- Enter units, dates and charges. Match units to the code’s time or per diem definition. Each line needs its own line-item date of service on outpatient claims.
- Add condition codes and the right TOB. Condition code 92 for IOP, 41 for hospital PHP. Check the type of bill: 013X, 076X, 077X or 085X.
Worked Example: FQHC Same-Day Medical and Mental Health Visit
An established Medicare patient sees the FQHC’s physician for a follow-up and then a licensed clinical social worker for a 45-minute psychotherapy session on the same day. Medicare allows these as two separate encounters.
| Line | Revenue code | HCPCS/CPT | Purpose |
|---|---|---|---|
| 1 | 052X | G0467 | Medical visit, established patient (payment code) |
| 2 | 052X | 99213 | Qualifying medical visit |
| 3 | 0900 | G0470 | Mental health visit, established patient (payment code) |
| 4 | 0900 | 90834 | Qualifying psychotherapy visit |
Payment math (2026): CMS MM14309 sets the FQHC PPS base rate at $207.72 per visit before the geographic adjustment factor (GAF). Assuming a GAF of 1.000 and charges above the rate, each encounter is paid at $207.72:
- 2 encounters × $207.72 = $415.44 total allowed
- Medicare pays 80% = $332.35
- Patient coinsurance 20% = $83.09 (no Part B deductible for FQHC visits)
If line 3 were billed on 052X instead of revenue code 0900, the claim could be returned or the mental health visit missed, and the FQHC would lose about $207.72 in allowed amount. Need help choosing between 90834 and 90837 for line 4? See our 90834 vs 90837 guide.

Common Revenue Code 0900 Mistakes and Denials
These are the errors we see most on behavioral health UB-04 claims, with the codes that usually come back. For a wider prevention plan, see how to reduce claim denials.
| Mistake | What you’ll see | How to fix it |
|---|---|---|
| Revenue code and HCPCS don’t match | CO-4 or CO-16 with a remark code | Use your payer crosswalk. For example, 90837 belongs on 0914 unless the payer says otherwise. |
| FQHC mental health line on 052X instead of revenue code 0900 | RTP or missed second encounter | Move G0469/G0470 and the qualifying line to 0900 and resubmit. |
| IOP claim without condition code 92 | RTP or claim processed incorrectly | Add condition code 92 and confirm the TOB. |
| IOP and PHP claims overlap | RTP, reason code 38363 | Check dates; a patient can’t be in both programs in the same period. |
| Revenue code on a CMS-1500 | Front-end rejection | Rebill on the correct claim form for the provider type. |
| FQHC G-code without a qualifying line | CO-16 | Add the qualifying visit (e.g. 90834) on the line after the G-code. |
| Service not covered under that revenue code or plan | CO-96 or CO-204 | Check coverage and the payer’s revenue code list before billing; appeal only with policy support. |
Texas Note: Texas Medicaid Behavioral Health Revenue Codes
Texas Medicaid behavioral health claims often use HCPCS H-codes, and the revenue code rules depend on the provider type and whether the claim goes to TMHP or a STAR managed care plan. Check the TMHP Behavioral Health and Case Management Services Handbook and the Inpatient and Outpatient Hospital Services Handbook in the Texas Medicaid Provider Procedures Manual before you build your crosswalk. FAQs
What is revenue code 0900?
Revenue code 0900 is the general classification for behavioral health treatments and services on institutional claims (UB-04 / 837I). It is used when a payer requires it, such as Medicare FQHC mental health visits (G0469/G0470) and psychiatric evaluations, or when no more specific 090X or 091X code fits the service.
What is revenue code 0905 vs 0906?
Revenue code 0905 is intensive outpatient services for psychiatric (mental health) care. Revenue code 0906 is intensive outpatient services for chemical dependency (substance use) care. Commercial and Medicaid payers often pay them as a daily per diem. Medicare FQHC and RHC IOP claims use 0905 with condition code 92.
Which revenue code is used for intensive outpatient?
Most payers use 0905 for psychiatric IOP and 0906 for chemical dependency IOP. Medicare hospital and CMHC IOP claims list each service on its own revenue code line, such as 0905, 0914 or 0915, with condition code 92. FQHCs and RHCs bill Medicare IOP on 0905. Always check the payer’s manual.
What revenue code is used for partial hospitalization?
Commercial and Medicaid payers often use 0912 (less intensive) or 0913 (intensive) for a PHP per diem. Medicare PHP claims list each service on its own line, such as 0904, 0914, 0915, 0916 or 0918, and hospitals add condition code 41. Confirm the current list with your MAC.
What revenue code does an FQHC use for mental health visits?
Medicare FQHCs report mental health visits G0469 (new patient) and G0470 (established patient) on revenue code 0900, followed by a qualifying line such as 90834. Medical visits G0466–G0468 go on 052X. Revenue code 0519 is used only on Medicare Advantage supplemental claims.
What is revenue code 0914?
Revenue code 0914 is individual therapy in the behavioral health family. It is used on institutional claims with individual psychotherapy codes such as 90832, 90834 and 90837. Group therapy goes on 0915 and family therapy on 0916.
Does revenue code 0900 need a HCPCS code?
Usually, yes. Medicare requires hospitals and CMHCs to report a valid HCPCS or CPT code on IOP lines, including 0900 (for example, 90791 or 90792). FQHCs need G0469 or G0470 plus a qualifying code. Critical access hospitals are not required to report HCPCS on IOP claims. Commercial rules vary by contract.
Is revenue code 0900 billed on a UB-04 or CMS-1500?
Revenue code 0900 is billed only on the UB-04 (837I institutional claim). The CMS-1500 (837P professional claim) has no revenue code field. If your provider bills on the CMS-1500, report the CPT or HCPCS code and place of service instead.
Get Revenue Code 0900 Right the First Time
Revenue code 0900 looks simple, but the right behavioral health revenue code depends on the payer, the program type and the HCPCS on the line. Build a crosswalk, check condition codes on IOP and PHP claims, and keep FQHC mental health lines on 0900. If behavioral health claims keep coming back, our medical coding services team can review your revenue code setup and fix the errors at the source. Book a free billing review.
